My problem was with remote users who had restricted shells. When logging in via ssh, they could not access the $HOME/.ssh directory. The problem lay with me for believing my fellow sysad. A single user had tried to modify his ssh identity files using 'cat >>' and had failed. Moral: never believe what the user tells you.
