I FINALLY just got this resolved.. The problem was traced to a corrupted /etc/path_to_inst file. I removed all entries in /etc/path_to_inst that was not a disk, dropped to prom mode, at the ok prompt typed boot -r, which recreated the /etc/path_to_inst file and all is now working as expected. This also fixed a problem with all of our t1 200's not being able to boot without a console.
