[Summary] Boot X4540 from USB/DVD

Thanks to input from Bryan Hodgson, I actaully went to the data center 
and had a look at the server itself and found that the USB/DVD device 
has usb power and external power. Even though the light was green on the 
USB device, it still needed the external power in order to be seen by 
the bios.

Once it the external power was satisfied the BIOS displayed options to 
boot from it.
