And the answer is:

Clean your CD Player. Most people suggested a termination or cabling problem.
I rechecked all of that to no avail. The winner was Mike Myers who suggested
cleaning my CDs. I realized the disks were clean, but the player was filthy.
Allsop's Compact Disc Laser Lens Cleaner worked like a charm.
