I got 2 people who stated that the only way the defect list changes is
through use of the format command's defect mechanism. 1 person
indicated that in addition to the format command, the OS will add a
defect if a disk get an error on writing a block if doing "slip
sectoring", or maps it if you are doing "defect mapping".
